Health Care for the Homeless
Partnership Health Center’s Health Care for the Homeless provides medical services for people experiencing homelessness. Health Care for the Homeless typically serves adults and/or families who stay in emergency shelters; sleep in makeshift shelters or on the streets; temporarily live with friends or family members; eat in soup kitchens; or visit drop-in centers or other sites providing temporary assistance.

Finding HelpWe also assist people and/or families experiencing homelessness in finding food, clothing, and shelter. We are a proud member of the Mayor's Committee on Homelessness and participate on several work groups designed to assist homeless individuals in the community.

Social Work
Partnership Health Center’s social work team supports established patients in coping with problems that can impact their health and wellbeing such as housing, safety, and food security.

Our social work team accomplishes this in a variety of ways from one-time appointments connecting patients with other resources in our community, to helping patients access public benefits, to short-term case management services.

Patient Centered Medical Home
Partnership Health Center is recognized by the National Center for Quality Assurance as a Patient Centered Medical Home, 2014 standards.
For PHC patients this means that we provide as many services as we can under one roof and routinely collect patient feedback about their experiences and any unmet needs. |
We use patient feedback to determine the next steps for program development. Over the years we have grown from a one-room medical office to a large community health center with fully integrated medical, dental, behavioral health, pharmacy, and other ancillary services.
