We can schedule a new patient appointment for you at any of our locations. Call today and we will do our best to get you in as quickly as possible.

Before Your Appointment

Complete your registration before you check in. Be sure to submit or bring in a photo ID, your insurance cards, and your proof of income to apply for a sliding fee scale discount.

​Our sliding fee scale (SFS) is available to everyone who qualifies, even people who have insurance.

​You may complete forms before your appointment.

Online Appointment Scheduling

Established PHC patients may now schedule an in person appointment through our online scheduling feature. Click here to schedule an appointment using our online scheduler.


What to Expect

We are committed to serving you as a patient-centered medical home. Your health care at Partnership Health Center focuses on your health goals, and you will work with a medical team to help you reach those goals.
​
Your medical care team includes your primary care provider (PCP), one or more family medicine resident physicians, nursing staff, and reception. Once you have established care with a PCP you can access PHC's menu of behavioral health services. 
If you are seeking dental care, we are equally committed to serving your needs as a team. Your dental care team includes your dentist, a hygienist, dental assistants, and reception.

Your Health History

The more we know about your health history, the better we are able to serve you—the whole you! Please tell us about any medications you are taking and provide records for any previous treatment you have had at other facilities. 

​We are happy to help you get your medical records. You can fill out a release before your appointment and bring it with you, or you can visit one of PHC's clinics for help filling out your release in person.

To register online click here.
Once your registration has been completed, we can give you access to our patient portal. This is a secure web-based service you may use to communicate with your care team, request appointments, and view your health records. Once you are enrolled, you will also be able to request pharmacy refills online and make payments towards your bill.

Sliding Fee Scale & Insurance Assistance

As a Federally Qualified Health Center, Partnership Health Center offers medical, dental, behavioral health, pharmacy, and lab services on a sliding fee scale. This means that you may be eligible for discounts depending on your household income and family size.  

When you check in for your appointment we will ask you for a nominal fee. Your balance after insurance payments and the sliding fee scale discount will be billed to you.

Find Us

We are conveniently located in downtown Missoula at 401 Railroad St. W. We are easily accessed from I-90 via the Orange Street Exit. PHC is also just blocks away from the Mountain Line Transfer Station, also located downtown.
This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

Partnership Health Center
provides language assistance services, including translated documents and oral interpretation, to individuals with limited English proficiency. We provide assistance in the following languages:
