Important Information on PHC Services:
*All patients and visitors will be asked several questions related to COVID-19 symptoms prior to entering facilities. We ask that you please arrive at least 10 minutes early to your appointment.
*Face coverings are required for all people inside PHC facilities.

- If you currently participate in individual and/or group behavioral health, please note that all visits have moved to telehealth visits (phone/video) until further notice.
